Are you a survivor of domestic abuse?

Your voice as a survivor of domestic abuse will help us to improve services, support, and training. By sharing your lived experience anonymously, we can help to shape future domestic abuse support for victims, survivors, and their families.

If you are 16 years or above and have been affected by domestic abuse, we would like to hear from you via our short questionnaire. The questionnaire will take around 10 minutes to complete. We thank you for your time and your input.

Please DO NOT use this questionnaire if you need support as personal contact details are not collected. If you need support, please use the Wakefield District Domestic Abuse Service.

If you are unable to complete the online form, or would prefer to speak to someone, please email the Domestic Abuse Engagement Team at  DAengagement@wakefield.gov.uk.

Support in sharing your experience

We know that describing your experiences of domestic abuse can be extremely difficult. If after completing the questionnaire you feel that you need support, please visit the ‘Getting Support’ section on the website, and contact the most appropriate service for you. There is also a PETALS support group available for women aged 18+.

Wider enquiries

Members of the public and professionals with enquiries should email the DAEngagement team at DAengagement@wakefield.gov.uk.

Please note this is not the route for referrals. Referrals should use Wakefield Domestic Abuse Support Service (Wakefield DASS).
