Identify staff and volunteers eligible for the vaccine

We are asking community-based social care services to help us to identify eligible staff and volunteers who have direct contact with service users (face to face) within their organisation by completing this survey:

Once we have details of the number of eligible frontline staff and volunteers in your organisation we will work with NHS vaccination services to identify and arrange appointments at one of our Wakefield vaccination sites.

When appointments become available we will contact you to request individual staff and volunteers details to enable the booking process. An example of the information required can be seen below.
