What is Get Active?

The Get Active Scheme makes it possible for nurses and GP's to refer patients for safe exercise that might improve their condition or quality of life.

This scheme puts clients in touch with a dedicated team of Health and Wellbeing Activators who will work with you to plan an activity programme suited to your needs.

The Health and Wellbeing Activators will provide you with a professional, understanding and confidential service.

Referrals can be made using the application form.

Who are exercise referrals for?

If your nurse or GP has made the referral, you are somebody whose health could improve by becoming more active. Many people with health concerns are not sure about which activities they can do safely, so this scheme will give you a chance to get the advice and support you need to become more active.

Weight management

The specialist weight management service is a combined 12 week programme of lifestyle changes and physical activity where you will be working with a team of health professionals with different skills and knowledge.

They work together to help you to deal with all the areas that can affect your weight for example eating, activity, behaviour, medical conditions.

Our team of health professionals include:

  • Activators
  • Dietitians
  • Physiotherapist
  • Medical doctors

Physical activity is delivered through Wakefield Council Sport and Active Lifestyles team. Sessions are delivered in Council sports and leisure facilities. They provide a safe and welcoming environment for you to exercise in.

You are required to attend a 1 hour exercise session which is mandatory and is sometimes held before or after a specialist dietitian group session. The physical activity instructors will assess your level or mobility prior to you starting an exercise session.

These group programmes run each week for 2 hours over 12 weeks and delivered by a dietitian. They involve group discussions, sharing of experiences and support which area very important part of the programme.

During the programmes we monitor people's progress, including changes in weight and general wellbeing. Weighing is done in private with your consent. On average, groups are made up of 12-15 people.
