Step 2 - creating a care and support plan

If following your assessment you have any unmet needs then we will work with you to create a care and support plan. This is your plan for what you want to do or achieve and  how you wish to spend your indicative personal budget (if you have one).

We will ensure that you are at the centre of the support planning process. This means that you will make choices about the services and care you receive. This is known as Self Directed Support.

Here are some examples of what might be included in a support plan:

  • care at home, such as help with bathing, cooking, getting around your community, helpful equipment like a personal alarm in case of falls
  • taking part in activities, Clubs and Social Groups
  • care away from home, such as day centres, respite care or short breaks in residential care
  • information and advice to help you stay safe and well

There are lots of options available to keep people living independently or to remain living in their own home. All these should be considered before making a decision to move into a care home.

We will give you a copy of your assessment and support plan. Your support plan will be reviewed each year to make sure that it still meets your needs.
