Guidance and help to manage your home
Do I qualify for help?
You qualify for help from our Supported Housing Team if:
- you live in the Wakefield district and
- you would benefit from guidance and help to manage your home.
How long is support provided for?
We aim to promote independence in our clients, so we only provide support for up to two years.
How much support we give you depends entirely on your individual needs and circumstances and we will assess this in the initial interview with you.
Once we have made this assessment, we will assign a Support Worker to you and we can agree a Support Plan to suit you and your needs. This Support plan details the support you need and how often we plan to visit you.
We can revise and change this support at any time to meet any change in your circumstances and your support needs.
