You need to fill in an application form for each person who wants a badge and send it to the address shown on the form. You can get a form in the following ways:

• Download and complete the application form (see 'Online Forms')

What do I need to include in my application?

Please can you include the following information in your application:

• your signature
• the £2 administration fee (cash not accepted)
• two passport photographs (must be under two years old)
• any other relevant information to support your application.

Once completed please can you return your application form to the following address:

It usually takes no longer than 28 days to issue a badge from when we receive the completed application form and a form completed by your doctor if necessary.

Blue Badge renewal

Your Blue Badge will expire at the end of three years and you will then have to re-apply for a new Blue Badge.  You can re-apply one month prior the expiry date highlighted on your Blue Badge.

You will need to complete a new application form, provide two current passport photographs (must be under two years old), as well as the application information outlined above.
