Targeted Youth Support (TYS)
Targeted Youth Support (TYS) is a multi-agency support programme for children and young people.
Our main purpose is to identify and offer support to children and young people (8-19 yrs) who are at ‘risk’ of substance misuse, teenage pregnancy, NEET (Not in Education, Employment or Training), crime and anti-social behaviour, homelessness, poor health and lack of positive activities.
We work in Wakefield with other agencies to achieve the vision set out in the Children and Young Peoples Plan which is to make sure that children and young people in the district are safe, supported and fulfilled.
The TYS provision works towards seven key delivery elements:
- Strengthening the influence of vulnerable young people and their families and communities, and their ability to create positive change.
- Identifying vulnerable young people early, in the context of their everyday lives.
- Building a clear picture of individual needs, shared by young people and the agencies working with them, contributing to the Common Assessment Framework (CAF) and other assessment tools that enable positive outcomes.
- Enabling vulnerable young people to receive early support in universal settings. Help all agencies to draw in extra help on behalf of young people, through better links between agencies and organisations.
- Ensuring vulnerable young people receive a personalised package of support, information, advice and guidance, and learning and development opportunities, with support for their parents or carers as appropriate. This should be co‑ordinated by a trusted lead professional and delivered by agencies working together.
- Providing support for vulnerable young people across transitions, for example moving on from school or from the support of one service to another as their needs change.
- Making services more accessible, attractive and relevant for vulnerable young people.
Working with partner agencies we aim to support this group of young people and their families by finding innovative, creative and inclusive ways of raising the aspirations of young people. We hope this will enable them to make a positive transition into adulthood.
Youth Inclusion Panel
The TYS has established a multi-disciplinary Youth Inclusion Panel to target resources at young people at highest risk of crime, anti-social behaviour, Teenage conception, NEET and substance misuse.
The Panel meets on a monthly basis to discuss the individual assessment outcomes from the nominations to offer the appropriate support plans for children, young people and their families.
