Physical Disabilities Team
The Physical Disability Team is part of Family Services. The team offers a service to people with physical disabilities to promote independence, health, safety and general well-being.
If you think that we may be able to help you please contact Social Care Direct, your first point of contact for care services (see under 'Also In'). They will refer your details on to us.
Depending on the information you have given we will:-
- Inform you whether you are eligible for a service
- Arrange to visit you in your own home to access your abilities and areas of need
- We can also offer an assessment for your carer/s who may be having difficulty or are at risk in their caring role
Assessing and Meeting Your Needs
The assessment will be completed with you and other people such as an Occupational Therapist, Physiotherapist and Nurses.
If you are eligible for a service we will then discuss with you with you the best way to meet your needs. This may involve the use of services such as Home Care or personal budgets as part of the new Self Directed Support.

We work closely with other services and may be able to refer you to:-
- Adaptations Assessment Team - for major adaptations or re-housing issues
- Wakefield District Housing (WDH) - for re-housing and property adaptations for WDH tenants
- Ability Centre - for advice about privately purchasing equipment to assist you
- Wheelchair Services
