Wakefield District has 5 Community Mental Health Teams (CMHTs) support people with severe mental health problems who live in the community, they are:

Our opening hours are 9am - 4.30pm Monday to Friday.
If no one is in the office there is an answer-phone. Don't worry about leaving messages as they are confidential to the team and can't be overheard by anyone else. Your call will be returned as soon as possible.
If you or someone you know needs help then you can contact Social Care Direct, which is the first point of contact for Social Services and is available 24 hours seven days a week (see 'Related Links').
Remember to let us know if you have any communication needs. We can arrange a translator or signing service if needed.

How we can help
The teams include staff from different professional backgrounds who have experience of working with people with mental health problems.
We offer services to people in the district with severe mental health problems referred by their doctor or other mental health professional. We offer an assessment:
- of the person's health and social care needs
- of the needs for anyone who is caring for the person if necessary.
Following this assessment, a care plan is developed with you which aims to provide the support and help you need.
