At your first appointment a member of the team will ask you to describe the problems you are experiencing.  An assessment will be made and if you would benefit from a service the CMHT can offer, you will be allocated a Care Co-ordinator who will develop a care plan with you and your carer if you have one.

Your care plan will describe the support you need and how this will be provided to you.  It will also explain who to contact in a crisis, information on what to do if the care plan cannot be provided and how to manage if your problem gets worse.  This care plan will be reviewed as often as you and your Care Co-ordinator think is necessary.

Your details will be confidential to the team providing support to you and will not be shared with anyone else unless you agree (unless you or another person is felt to be at undue risk).
