We have established links with the Hospital Occupational Therapists in our area so that if you need equipment or adaptations to your home before you are discharged, we can arrange this quickly and without increasing your stay in hospital. If you need more help to enable you to do everyday tasks, they can also assist you.

We also link with other Occupational Therapy Services in the community. These include:-

• the Family Services Community Care Occupational Therapists for the Elderly,
• the Resource Centre Therapy Team,
• the Community Physical Disability Team
• Mental Health and Learning Disability Services.

We do assessments as soon as possible. We aim to do them within 28 calendar days. We deal with people due to leave hospital, who are terminally ill or who are a risk to themselves or others first - usually within 5 working days.
