In order to claim the nursery education funding a Parent Declaration form must be completed and signed by the parent/carer. These forms will be given to you by your provider of nursery education and must be returned to them. Full instructions on the completion of the form and the conditions of Nursery Education Funding can be found on the reverse of the Parent Declaration form.
If your child attends more than one eligible provider then you must complete a Parent Declaration form for each provision the child attends.
The forms should be completed in the first half of the term unless you are only claiming the second half terms funding only.
