Apply for a death certificate

To apply for a death certificates you will need  to provide the following information:

  • Name and surname of deceased Age at death or date of birth (if known)
  • Date of death (or approximate year if exact date not known)
  • Where the death occurred (town or village if exact place not known)
  • The address to which the certificate is to be posted and a telephone number where we can contact you.

How to apply for a death certificate

Online
Complete the death certificate online application form using the link below. 

Download an application form
You can download the certificate application forms from "Downloads"

In person
Visit the appropriate register office and the certificate will be prepared while you wait.

By phone
Call the appropriate office and pay by debit or credit card. The certificate(s) will be posted to you the same day.

Email
Email the appropriate office with your contact details and a member of staff will telephone you to take payment by debit or credit card and the certificate(s) will be posted to you the same day.
