Criminal Records Bureau Disclosure
An Enhanced Criminal Records Bureau Disclosure is required as part of a Private Hire Driver Licence application. This form is not avaliable to download.  A form can be obtained from the Licensing Office (see 'Related Links' for contact details).

Driver Standards Agency (DSA) Private Hire/Hackney Carriage Assessment Certificate must be produced on submission of the following:-

• on first application for a licence
• on lapse of a licence for 12 months or more
• at other times if the Council considers it necessary

DSA Wheelchair Assessment Certificate
You will be required to produce this certificate if the vehicle you intend to drive is wheelchair accessible. The testing station within the Wakefield District for Wheelchair Assessment is DSA Pontefract, Baghill Station, Station Lane, Pontefract WF8 1RB. (There are other testing stations throughout the country where the Wheelchair Assessment is available. For more information contact the DSA.
